Each company keeps its own details.Add a company

Each form is the issuer’s own file, filled from one set of company details. The declarations stay yours to answer, and the affidavit prints for you to sign in person.

Enter your company once. Every form reads from it.

  • Start from CIPC. Type your registration number and KuduDoc brings in the company name, registration details and directors that CIPC holds.
  • Add what CIPC does not hold. The trading name, addresses, tax numbers, your CSD supplier number, your tax compliance status (TCS) PIN and its expiry, your financial year-end and a contact person.
  • Change it once. Each detail shows where it came from and when. Change it once and every new form uses it; a form you have already signed keeps what it said.
  • Run more than one company? Each keeps its own details.

The same details open the company file, where you keep the company’s records together and KuduDoc watches the annual return date.

SBD 1, SBD 4 and SBD 6.1, filled on the bid’s own pages.

Upload the bid document. KuduDoc finds the standard bidding documents in it and fills them where they sit, because SBD 1 asks for bids on the official forms, not retyped. No bid document yet? Start from National Treasury’s current copy. The filled forms go in with a tender’s returnable documents.

SBD 4 changed on 1 August 2026

National Treasury replaced SBD 4, the bidder’s disclosure, from 1 August 2026 for departments, constitutional institutions and the public entities listed in Schedules 2 and 3 to the PFMA (PFMA SCM Instruction 12 of 2026/27). Where the bidder’s directors, members or owners have an interest in a related enterprise, the new form asks for each one registered on the CSD, with its CSD supplier number and status, and says that failing to disclose an active one leads to disqualification. KuduDoc fills whichever version a bid carries and tells you which one it is. Last checked 25 September 2026.

Sign where the bid allows it

  • The ECT Act lets the institution receiving a document decide how it accepts it (s4(2)), so read the bid’s instructions first.
  • Where it accepts electronic signatures and does not ask for an advanced one, sign with an ordinary electronic signature: each person who must sign gets their own link and signs with a free KuduDoc account; the signed set is sealed. KuduDoc signatures are ordinary electronic signatures under the ECT Act.
  • Where it asks for ink, print the filled forms, sign in ink and keep a sealed scan of what you sent.

The signed forms stay with the bid number and closing date: the rest of the bid, in one company file. And see which documents still need ink.

The B-BBEE sworn affidavit, filled here and sworn before a commissioner.

An exempted micro enterprise (EME), or a qualifying small enterprise (QSE) at a level its template lists, can declare its B-BBEE status in a sworn affidavit on the template the issuer publishes. KuduDoc fills the current template from your company details and gives you a print-ready copy to swear. For an EME, CIPC says a commissioned affidavit serves as its B-BBEE certificate.

Pick the template: general, or your sector code.

TemplatePublished byKuduDoc has

You choose your sector code and your size. Each template shows the turnover band it covers before you fill it.

What KuduDoc fills, and what you declare

KuduDoc fills

  • The deponent’s full name
  • ID number
  • Capacity: member, director or owner
  • Enterprise name
  • Trading name
  • Registration number
  • Tax number, where the form asks
  • Address
  • Type of entity
  • Nature of business
  • Financial year-end

You declare

  • The ownership percentages the form asks for
  • The revenue statement
  • The level box

KuduDoc never ticks the level for you: the template lists the levels it covers, and you tick yours. Last year’s figures sit beside each box for reference.

Sign it in front of a commissioner of oaths

You sign the affidavit in the commissioner’s presence (Regulations Governing the Administering of an Oath or Affirmation, 1972, reg 3(1)). KuduDoc never sends a B-BBEE affidavit for electronic signature: it gives you a print-ready copy.

The commissioner certifies below your signature with the date, place, their name, business address and designation, and charges nothing for administering the oath (regs 4 and 6).

Take the printed affidavit to a commissioner of oaths of your choice: see where to have it commissioned, and which documents still need ink.

Sealed, kept and counted

  1. Photograph or scan the stamped affidavit. KuduDoc reads the commissioner’s date and shows you the line it read, for you to confirm.
  2. Sealed and kept. Then it seals the scan and keeps it with your company details. Keep the signed paper too: a sealed scan does not replace a paper original where one is required.
  3. Counted, and filled again. The affidavit is valid for 12 months from the date the commissioner signed it (CIPC’s template). KuduDoc counts from that date and emails you 90, 60, 30 and 7 days before the last day, by default — or on the days you choose. When it is time, next year’s affidavit is filled in for you to check and swear again.

KuduDoc watches your B-BBEE certificate and tax compliance status PIN the same way.

When do the affidavit’s 12 months end?

A commissioner signed on 15 March 2026: the affidavit’s 12 months end on 14 March 2027.

In your account

Date the commissioner signed 15 March 2026

The count ends the day before the same date a year later; a start on 29 February ends on 28 February.

Its 12 months end on 14 March 2027.

Keep it in KuduDoc and the default reminders fall on these days, or on the days you choose:

  1. 14 Dec 202690 days before
  2. 13 Jan 202760 days before
  3. 12 Feb 202730 days before
  4. 7 Mar 20277 days before

CIPC’s EME template: valid ‘for a period of 12 months from the date signed by commissioner’.

Licence disc and driving licence renewal forms, filled from your household details.

  • Licence disc: the MVL2 notice, or form ALV.

Bring the renewal notice (MVL2) if it arrived. If it did not, you complete form ALV, the application for licensing of a motor vehicle, and KuduDoc fills it from your vehicle and owner details. Renew at your registering authority, at a post office that licenses vehicles, or online on NaTIS, which couriers the disc. For a company vehicle, gov.za also asks for the business certificate, the proxy’s ID and a letter of proxy: KuduDoc keeps them with the vehicle. The disc is renewed every year, with 21 days’ grace from its expiry date; KuduDoc watches the date.

  • Driving licence card: form DL1.

For the card, gov.za lists form DL1, the application for renewal of a driving licence card, and form NCP, the notification of change of address or particulars. Take your ID and a copy, black-and-white ID photographs and proof of address to the driving licence testing centre; the eye test is done there or by an optometrist of your choice. gov.za asks residents to apply online and to renew four weeks before the card expires. KuduDoc fills DL1 and NCP and watches the date printed on your card.

  • Proof of address.

If the utility bill is not in your name, gov.za asks for an affidavit from the bill’s owner, with the bill attached. KuduDoc prepares it for them to sign in front of a commissioner of oaths.

KuduDoc does not renew anything: you renew with your licensing office or online. Keep the car’s papers together, and see how long each licence and certificate lasts.

The issuer’s own form, dated and linked.

Each form is the issuer’s own file, unchanged except for what you fill in. KuduDoc records the SHA-256 fingerprint of each blank as the issuer published it and shows it in the form’s details, so you can compare it with the issuer’s file. When an issuer publishes a new version, KuduDoc moves to it and tells you if a draft of yours is on the old one.

Questions people ask

Compare plans →

Where do I get a B-BBEE sworn affidavit template for an EME or QSE?

From the issuer. The dtic publishes the general affidavits for exempted micro enterprises (EMEs) and qualifying small enterprises (QSEs), CIPC publishes the general EME affidavit, and the B-BBEE Commission publishes the gazetted sector templates. KuduDoc keeps the current version of each, dated and linked to the issuer’s copy, and fills it from your company details.

Do I still need a commissioner of oaths if KuduDoc fills in the affidavit?

Yes. You sign the affidavit in the commissioner’s presence (Regulations Governing the Administering of an Oath or Affirmation, 1972, reg 3(1)), so KuduDoc gives you a print-ready copy instead of sending it for electronic signature. Afterwards, photograph or scan the stamped page and KuduDoc seals it and keeps it with the commissioner’s date.

When does KuduDoc remind me to renew the affidavit?

Before its 12 months run out. The affidavit is valid for 12 months from the date the commissioner signed it, so KuduDoc counts from that date and emails you 90, 60, 30 and 7 days before the last day, by default — or on the days you choose. When it is time, next year’s affidavit is filled in for you to check and swear again.

Which SBD forms does KuduDoc fill?

SBD 1 (invitation to bid), SBD 4 (bidder’s disclosure) and SBD 6.1 (preference points claim form under the Preferential Procurement Regulations, 2022), and their municipal MBD versions. Upload the bid document and KuduDoc fills them on the bid’s own pages, because SBD 1 asks for bids on the official forms, not retyped. The yes-or-no declarations are yours to answer.

What changed in the SBD 4 form in 2026?

National Treasury replaced SBD 4, the bidder’s disclosure, from 1 August 2026 for departments, constitutional institutions and the public entities listed in Schedules 2 and 3 to the PFMA (PFMA SCM Instruction 12 of 2026/27). Where the bidder’s directors, members or owners have an interest in a related enterprise, the new form asks for each one registered on the CSD, with its CSD supplier number and status. Failing to disclose an active one leads to disqualification. KuduDoc tells you which version a bid document carries.

Can I sign the SBD forms electronically?

If the bid allows it. The ECT Act lets the institution receiving a document decide how it accepts it (s4(2)), so read the bid’s instructions first. Where the bid accepts electronic signatures and does not ask for an advanced one, sign in KuduDoc with an ordinary electronic signature. Where it asks for ink, print the filled forms, sign in ink and keep a sealed scan of what you sent.

Does KuduDoc submit the bid for me?

No. You submit the bid to the institution through its portal, by email or in its bid box, as the bid says. KuduDoc fills the forms, keeps the signed copies in your company file with the bid number and closing date, and reminds you before the affidavit and your certificates run out.

Where do I get the licence disc or driving licence renewal form?

For the licence disc, bring the renewal notice (MVL2); if it did not arrive, you complete form ALV. For the driving licence card, gov.za lists form DL1 and form NCP (notification of change of address or particulars). KuduDoc fills these from your household details for you to take to the licensing office, and watches both expiry dates.

Sources: the oath regulations (justice.gov.za), the ECT Act (gov.za), National Treasury’s standard bidding forms, the dtic, CIPC and the B-BBEE Commission templates, and gov.za’s licence renewal pages for the licence disc and the driving licence card. Last checked 25 September 2026.
